Who could use cardiac rehab? A county map
How common are the heart conditions that qualify patients for cardiac rehabilitation, county by county. Pick a measure; hover a county to compare all of them.
National cardiac rehab participation over time
Share of eligible Medicare patients who attended at least one cardiac rehab session, from published national studies. Studies differ in which events qualify and how patients were sampled, so treat the line as a rough trend. Horizontal bars show multi-year study periods.
About the data
Eligibility. Medicare covers cardiac rehab after a heart attack, bypass surgery, angioplasty/stenting, heart valve repair or replacement, heart or heart–lung transplant, stable angina, and stable heart failure with reduced ejection fraction. The CR-eligible rate counts Million Hearts primary qualifying events (heart attack, bypass, PCI, valve procedure, transplant) among Medicare fee-for-service beneficiaries; the hospitalization rates are broader proxies for the same population.
Caveats. Medicare measures cover fee-for-service beneficiaries 65+ only (Medicare Advantage enrollees are not included). Hospitalization rates are age-standardized and spatially smoothed by CDC. Coronary heart disease prevalence is a modeled self-report estimate for all adults; PLACES has no estimates for Kentucky or Pennsylvania in this release, and reports Connecticut by planning region, which doesn't match these county shapes. CDC labels the CR eligibility period both 2018–2019 and 2019–2020. Gray counties have no data or were suppressed for small numbers.
Small counts. CDC publishes rates without counts. To flag unreliable rates, the page estimates each county's CR-eligible patients as the eligibility rate × the county's 2019 Original Medicare enrollment (CMS Geographic Variation PUF, all ages). CR measures in counties with fewer than 50 estimated eligible patients are hatched; at that size a single patient moves the rate by 2 or more points. Most of the 70%+ participation rates are in such counties: Sheridan County, Nebraska's 99.6% rests on roughly 9 patients.
